    Push Fades

So, if I'm hitting a push fade all round then what is the most likely cause of this? Anybody have any ideas?

You have an inside path (which is great) but have an open club face at impact. Turn your left (top) hand clockwise until the PF's disappear.

what club? all of them? some fade more than others? are they big pushes that tail right (if you are a righty) or do they start a wee bit right and turn more throughout?
driver, ball flight start line ~85% of face direction. you could have a way open face and outside in swing path as well.
push fades are sometimes tiny slices.
if you do as BC says you will find out quickly if the path is inside out or outside in if those "push fades" start to turn into pull hooks. it's a good start, but i suggest:
    see a pro for a look-see.

i have a 3-9* in to out. longer with woods short with irons. i have this problem with woods. the source is the cpm on the shaft is slightly too strong preventing to slightly close the face relative to path to make it come back. tested shafts the other day that are still stiff but weaker cpm and the face closed. try that. all reg/ stiff / xstiff are not the same cpm. they vary a lot.
