He then takes the ball off the tee and puts it on the grass and hits his ball to the fairrway.

Here's my guess:

1 stroke for the whiff
1 stroke for moving the ball since it's now in play
2 strokes for playing the ball from the wrong spot (these 2 strokes over-ride the previous 1 stroke penalty)
1 stroke for hitting his shot to the fairway

He's hitting 5 from the fairway.
