1) What is the affect of a shaft extention on iron lie angle? is there a ratio? say 1/2in equals 1deg
2) Can clubs be bent more than once? can they be bent back to original lie?

You are correct that 1/2 inch will increase your lie angle by 1 degree more upright
Yes you can bend a club more than once. It is easier to bend a forged club then it is a cast club. Forged clubs can bend easily 4 degrees where cast clubs can usually only bend about 2 degrees.

A total of two bends, once up then back, isn't going to fatigue the metal enough to matter.

I strongly disagree on all counts. The 1/2" to 1 deg,ratio does not apply in any circomstance, especially when using extentions,You are effectivly stiffning up the butt section and softning the tip section simotaniously and establishing a whole new bend profile to the shaft.The only way to establish the proper lie is a lie test.
There are cast clubs that can be bent 3 to 4 deg, no problem. It all depends on the type of steel and the quality of the manufacturing.
